Active Care continually works with insurance contracts to provide the greatest access to our patients. We are currently contracted with the following insurances companies:

As a courtesy, we at Active Care are happy to verify outpatient physical therapy insurance benefit coverage for our patients prior to initial visit. If you have any questions regarding insurance verification or billing, please contract our front desk staff at [email protected].
*Please bring insurance cards, as well as all clinical forms (post-op, MRI etc…) to your first appointment.
